Combinations first appeared in Riding the Storm Out, where after using a multi-tool on the Omnitrix, Ben caused the device to transform into Grey Arms. He was later turned into a combination of all 10 of his available transformations as well as himself, known as Amalgam Ben. Not using the Omnitrix for a prolonged period of time afterwards seemingly fixed the glitch.
In Funhouse, Zombozo subjected Team Tennyson to an illusion of Ben switching between his available transformations, giving the false impression of proper combinations.
In Tales from the Omnitrix, Ben told a story involving another combination of Kevin's aliens. In the story, the combination was involuntary and caused by Ben transforming into Shock Rock. He was also able to change the attributes of the transformation by changing into Four Arms. Likewise, Ben's reversion from his alien forms returned Kevin to normal.
In Ben 10 Versus the Universe: The Movie, Vilgax used the Antitrix to transform into AntiVilgax, which like Monster Kevin, is a combination of all the Antitrix aliens and the user. He later inserted the Omnitrix Key into the Antitrix to become Alien V, a combination of his own DNA and a Celestialsapien's. Once he was defeated, Azmuth used the Omnitrix to return Vilgax to his usual self.

The Omnitrix's combinations can be accessed through causing the device to malfunction[1] or by entering the DNA Pods inside the device consecutively.[2]
Antitrix[]
The Antitrix's combinations can be accessed either via magic,[3] proper manipulation of the device's controls,[4][KT 1] or by inserting the Omnitrix Key.[4]
